Dear USA Dairy Producer: Thank you for your interest in this wage survey. It grew out of receiving so many questions about what other dairy farmers pay. There is a window between May 10 and the end of July 2000, to participate, and we hope you will choose to. The data you provide will be kept confidential and only released in summary form.

In order to accomodate different needs, besides the form you can fill out right here in this page, you can if you prefer, download either (1) Microsoft Word 97 document, or (2) a PDF format document (for which you will need to download a free PDF reader). Important instructions for dairy farmers filling out Web form: Please answer all questions for April 2000, unless otherwise noted, and only for non-family dairy personnel. Fill out part one only once (questions 1-12). For part two (questions 13-22), answer for a specific non-family employee: do not average among several employees. AOL does not seem to support forms, so AOL clients, please download the paper verion above--sorry :-(.

If you do wish to include additional employees (and that would be great if you so desire), please use the following approach: First, fill out part one (general dairy questions) followed by part two, specific to one of your employees. Press the submit button at the bottom of the form. Only after having pressed the submit button at the bottom of the page, re-enter information for the new person you want to include, beginning with part two (leave part one as is). Make sure to press sumbit after completing each new employee you want to include.

Two last important points. (1) The computer will warn you that you are submitting information via E-mail when you press the "submit query" button. The data you provide will be kept confidential and only released in summary form. (2) Sometimes, users have not set up their browsers to use forms. This can usually be fixed in a couple of minutes by talking to your Internet provider, who will instruct you on how to set your "mail preferences." If this is not possible, feel free to use the forms provided above.
